Welcome to 4-ACP

Project title: Implementation and evaluation of an integrated care planning and care coordination intervention in primary care for people living at home or in a care home at risk of deteriorating or dying: mixed-method study across Scotland.

 Outline: GP practices in 4 Health Boards are taking part in case studies.

Aim: Improve anticipatory care planning (ACP) in Scotland by using national resources for professionals and the public to support primary care teams and care home staff in helping people be more prepared for future changes in their health by planning ahead with their health and care team.

Project team: Principal Investigator is Kirsty Boyd (Reader in Palliative Care); Bruce Mason (bruce.mason@ed.ac.uk)  and Anne Canny are our researchers.
